Require SEND Transition Plans from age 14 to 25 to prepare for adult life
Require Local Authorities to provide all young people with SEND with a Transition Plan from age 14 to 25, covering education, employment, independent living, health and community participation, to improve preparation for adulthood and ensure consistent support during the transition to adult life.

Background
Too many young people with SEND face a "cliff edge" when moving into adulthood, with inconsistent support for education, employment, independent living, health and community participation.
We call on the Government to require a statutory Transition Plan for every young person with SEND from age 14 to 25. Plans should be reviewed regularly & set out clear goals and support to help prepare for adult life.
This could improve independence, wellbeing, employment opportunities for SEND young people.
